Surgical and Cosmetic appointments must be scheduled by phone.
We do not participate with any form of Medicaid.
Insurance participation may vary by plan. Please verify coverage with your insurance company or our office before booking.
Before Your Visit
- Referrals: Patients must obtain referrals if required by their plan
- Insurance: Please ensure your insurance information is up to date with our office
- Financial Responsibility: If your plan is out-of-network or doesn’t cover your visit, you may be responsible for payment
